For older adults, continuing to monitor care after release from a facility is crucial in ensuring a full recovery. With hospital-to-home transition care, ComForCare Home Care Scottsdale, Arizona can help to transfer your loved one from a facility, such as a hospital, to an assisted living community or to their home. Vital Palliative Scottsdale Arizona
Our mission is to exceed the ordinary as we collaborate with healthcare providers to help those with serious conditions and illnesses, by providing assistance, education and support to enhance quality of life.
Palliative Care is a relatively new specialty offered to individuals with serious and chronic illnesses such as congestive heart failure, cancer, chronic obstructive pulmonary disease (COPD), AIDS, kidney failure and Alzheimer’s.
The Palliative Care Team will work side by side with a patient’s Primary Care Physician, or Specialty Physician, to appropriately manage a patient’s pain and symptoms, while providing social support and resources to the patient’s family and friends.
