Our Easy Process

Step 1 - Visit
After speaking to our Hospice Advisor to help answer questions you may have, they will set up a time to meet with you and your family to discuss all details and complete paperwork.

Step 2 - Medical Records
We will gather all records necessary to start the process to make things as easy as possible on you.

Step 3 - Nurse Assessment
Our nurse will call and schedule an assessment on a time that works for you and your family.

Step 4 - Admission
Nurse will come to do the assessment of the patient. If appropriate she will do an admission for hospice.

Step 5 - Medical Supplies
Once the admit is completed, medical supplies and equipment are ordered and will be delivered to your home on a time that works for you and your family.

Step 6 - Meet Your Team
Once admitted you will meet your team of professionals who will support you and your family’s needs. The team is made up of an RN Case Manager, Licensed Nursing Assistant, Medical Social worker, and a Chaplin should you choose.
